If a minor is traveling without one or both parents the absentee parent(s) it is highly recommended to have the adsentee parent(s) sign a statement stating that the child is allowed to leave the country.  I have provided samples below.  You may not even be asked for the form but better safe then sorry.  If you are unable to prove the child can leave the country Carnival® can deny boarding without refund.

The forms are in word format and can be changed to suit the situation. Once opened select File from the menu > Save As > save to a location on your computer.  Some have difficultly getting the forms to load.  If this is the case then instead of clicking to open click the right mouse button and select "Save Target" this will allow you to download directly to your hard drive.
